Abstract

  Background and Objective : Socio-economical and scientific development has brought about rapid increase of elders and changed old traditions. Quality of life of elders with good wife’s, children's and friend’s support is higher than that of others. Hence, we aimed at determining people‘s attitude toward elders in Golestan province, 2009.

  Materials and Methods : This cross-sectional study was carried out via convenience sampling on 300 people, who resided in four cities of Golestan province and aged 13-60 years, having at least one elder around. Data was collected by kogan questionnaire and and analyzed by Kruskal-wallis and mann-whitney.

  Results : The mean age of the subjects is 24.8. They are Fars (53%) and Turkmen (42%). There is significant relation between attitude toward elders and Variables such as age (P=0.003), ethnicity (P=0/000) and education (P=0/01. Men and women younger than 25 years and people with higher education have more negative attitude. Turkmens have higher positive attitude toward the aged.

  Conclusion : Since more negative attitude toward the aged are reported among teens and highly educated adults, we recommend officials for performing some essential interventions to improve this attitude .

Abstract

  Background and Objective : Evaluation is considered as an integral component of academic planning system and the process of determining and providing necessary information about desirability of development program targets. The aim of present study was comprehensive evaluation of Islamic Azad University of Gorgan based on CIPP Model to provide useful information to help people to judge and improve the curriculum and develop the policy of programs .

  Materials and Methods: The type of this study is educational evaluation. First, for conducting the evaluation, four domains of CIPP model including context, input, process and product was divided into more detailed parts, then primary standards were developed based on viewpoints of deans, head of department and faculty members of the university. After that, based on primary standards, measurement tools were created and administrated.

  Results: The results showed that the average score of context, input, process and product from the perspective of faculty members, students, head of department, university administrators, professionals and graduates, is 2.55, 2.714, 3.208 and 2.91, respectively, which indicates the relative desirability of the university in these four factors. The total score of four factors is 2.845 indicating the relative desirability of the university in general.

  Conclusion:­ Given the youth of Gorgan Azad University and­ low proportion of faculty members with full professor and associate professor degree, this level of desirability is acceptable.

Abstract

  Background and Objective: Body image has a remarkable affect on people's lives, their self-esteem, mental well being and health. The aim of this study was to survey the body image of faculty members and staff engaged in schools of Golestan University of Medical Sciences.

 Materials and Methods:­ In this descriptive-analytical study­(2008),­using census, the subjects are educational, clerical and ordinary staff of all faculties of Golestan University of medical Sciences.­­ The instrument was a­valid and reliable­(r=­0.71)"body­ image"­questionnaire. The data related to variables such as gender, type of work, appearance, height and weight" were analyzed using analytical and descriptive statistics.

 Results: Of 225 participants, 63.6% of them have good, 33.6% moderate and 3.1% had a bad body image. There is significant relation between body image and variables such as gender (P=0.003), appearance (P=0.001) and height (P=0.027), but it is not significant for type of work, school and weight with body image.

 Conclusion:­ With regard to the importance of body image on mental health and because of constant interaction among different staff, having a positive body image may be effective in providing high quality at work. Therefore, applying some strategies to promote a good body image is necessary.

Abstract

  Background and Objective : Adolescence is a critical period of life accompanied by a high prevalence of anxiety disorders, which can be reduced by acquiring some special skills. The aim of present study was to investigate the effect of anxiety management skills on Students’ Life Satisfaction in one of the Girls’ High schools in Gorgan, 2010.

  Material and Methods : We conducted this pre-experimental study on 60 girl’s students , from a high school in Gorgan, selected via convenience sampling.­­We asked the subjects to fill out the demographic data sheet and Dinner life satisfaction questionnaire, and divided them randomly in two groups of intervention and control. For intervention group, a class of anxiety management was performed.­­To compare the means, we used independent t- test and paired t- test.

  Results: The results show that there is significant difference between the life satisfaction of before and that of after education­(P=0.028­), but it is not true for control group.

  Conclusion: Learning anxiety management skills can increase the rate of life satisfaction in Adolescents therefore, it can help them to overcome many social problems.

Abstract

  Background and Objective : One of the important factors for safe Parturition of women is considering the birth intervals. It seems that knowing about effective factor on behavior and action of people is good for health policy. Thus, we aimed at studying the knowledge, attitude and practice of married women, referring to Gorgan’s health centers in 2008, about contraception methods.

  Material and Methods : We conducted this descriptive-analytic study on 275 married women, selected via convenience sampling, referring to six health centers in Gorgan. The Data was collected by a researcher-made questionnaire (α=0.8) including four parts of demographic, knowledge, attitude and practice, and analyzed by SPSS software, using descriptive statistics, correlation and Chi Square.

Results: The main reported contraceptive methods are coitus interruptus (36.36%) and condoms (20.72%).­ There is significant relationship between knowledge and occupational status (P=0.011), knowledge and educational level­ (P<0.05), ­­practice and educational level­ (P<0.000), and between attitude and occupational status­ (P=0.012).

  Conclusion: Increased level of knowledge is one of the essential components of improving level of the health and hygiene. The main point for family planning should be stress on heightening of educational level.­ Strengthening the knowledge and changing the attitude may lead to women selecting the safe contraceptive method and modifying ­their fertility behaviors.

Abstract

  Background and Objective: Multiple Sclerosis ( MS) results in a range of unpleasant, unpredictable and physically disabling conditions. Thus, health promotion programmes are needed and Peer support groups have been shown to play a role in health promotion. The present study was conducted to evaluate the effectiveness of peer groups on promoting physical health status in MS patients.

  Material and Methods: This before and after quasi-experimental study was conducted in Tehran MS Association (2010) on 33 patients participated in three peer support groups: 10 in male group, 11 in female group and 12 in mixed group. Eight weekly sessions, each taken 2 hours, were held. The patients were asked to fill out the Physical aspect of "Multiple Sclerosis Quality of Life Inventory" with 78 items (α=0.93)­ in pre of intervention, two weeks after it, post of intervention and one month later.

  Results :­ The Results show that peer support groups promote the physical health status (p=0.001). In each three groups, there is a significant difference in physical health status in patients from before of intervention until one month after (p=0.001). This change is increasing from start of intervention until the end of it­ and then is reducing up to one month after intervention.

  Conclusion: The results show that peer groups can promote the physical health of patients with MS therefore, with the benefits of peer group, it is possible to promote the quality of care for MS patients, which in turn enhances the patients′ quality of life.

Abstract

  Background and objective : Low blood pressure and Muscle cramps are common complications of Hemodialysis. One approach that has recently been proposed to prevent this complication is the combination of sodium and ultrafiltration. The purpose of this study was to investigate the effect of of sodium and ultrafiltration profile on some of the common complications during Hemodialysis.

  Material and Methods: In this crossover clinical trial study, 22 Hemodialysis patients referred to Panje-­Azar Hospital in 2012 were divided randomly into two groups. For each group, two treatment protocols were conducted, six hemodialysis sessions. The intervention protocol was a linear sodium dialysate and linear ultrafiltration. In routine Protocol, both sodium dialysate and ultrafiltration were considered constant. Using chi-square test and relative risk, the data was analyzed (P<0.05).

  Results: The mean age is 54.73 ± 11.21 year and 59.1% of them are females. The incidence of hypotension and muscle cramps in the experimental protocol procedure is significantly decreased compared with that of control group (P<0.05), but the incidence of headache and vomiting is not significant (P<0.05).

  Conclusion: Because sodium and ultrafiltration profile is simple and cost-free and reduces the incidence of complications during dialysis, we recommend using sodium and UF profile instead of routine one.

Abstract

  Background and Objective: anemia is one of the main problems of chronic renal failure patients undergone Hemodialysis and erythropoietin is not effective for these patients. Since Vitamin C deficiency in Hemodialysis patients is common, this study was performed to compare the effect of intravenous and oral ascorbic acid consumption on anemia in Hemodialysis patients.

  Material and Methods: this clinical trial was conducted on 75 patients, selected via convenience sampling and including criteria (hemoglobin, 11mg/dl ferritin, 100ug/L transferring saturation, < 20%). The subjects were randomly assigned to three equal groups of control , oral ascorbic acid (POAA) and intravenous ascorbic acid (IVAA) After each every-other-day dialysis performed for eight weeks, the patients of IVAA group were given 300mg intravenous vitamin C and the ones of POAA group given 500mg vitamin C orally. The Plasma iron with chemical method, total iron-binding capacity (TIBC) and ferritin level with indirect method, hemoglobin with cell counter and transferring saturation (TSAT) with related formula were measured. Using a checklist, the data of demographic, laboratory results and side effects were recorded and analyzed by SPSS-13, using ANOVA and scheffe post-hoc test (p≤ 0.05).

  

  Results: the baseline hemoglobin, serum iron, ferritin, TIBC and TSAT in three groups were not different significantly. After intervention, the hemoglobin level of IVAA group( 20.1g/dl ) and in POAA group (0.99g/dl) were increased compared to control group(P>0.001). the mean of hemoglobin in IVAA was significantly more than POAA (P>0.05).

  

  Conclusion: based on the results, the use of oral and intravenous vitamin C can be effective in the anemia caused by iron deficiency, especially vitamin C injection that is more beneficial.

Abstract

  Background and Objective: ­ ­Delirium is a kind of fluctuating cognitive­ destruction , disorientation and ­a common problem for inpatients, which leads to increased mortality, prolonged hospital stay, increased cost and decreased rehabilitation.­ This study evaluates the incidence of delirium and its associated risk factors in open heart surgery.

  Material and Methods: This descriptive-analytical study was conducted ­­in six month (2011), via convenience sampling, on 404 patients undergoing elective cardiac surgery in the heart center of Fatemeh Zahra in Sari, Iran. Using Neecham questionnaire and demographic checklist, delirium was evaluated from the first till the fifth day of surgical operation. We used survival analysis (Kaplan-Meier and Cox regression) to­ analyze the findings, using SPSS software version18.
Results: The incidence of delirium is reported 28.5%. There is a significant relation between the incidence of delirium and variables such as age , lower education, rural, history of kidney failure, heart failure, stroke, hearing disorder, atrial fibrillation, increased serum potassium and increased serum urea.

  Conclusion: ­ According to the results, screening for d elirium is a matter of the utmost importance . With attention to individual profile and preoperative factors related to delirium, it is necessary to train elder and lower education patients, to maintain electrolyte balance before operation and to eliminate sensory deprivation in order to lessen the incidence of delirium.

Abstract

Background and objective: Hypotension and Muscle cramp are the common complications of Hemodialysis. One approach that has recently been proposed to prevent this complication is the change in the concentration of sodium and ultrafiltration. The purpose of this study was to investigate the influence of sodium dialysate variation and ultrafiltration in preventing hypotension and muscle cramp during hemodialysis process.
Material and Methods: In this clinical trial study, 44 Hemodialysis patients were divided randomly into four groups. For each group, four treatment protocols (six-session Hemodialysis) were conducted. Protocol A: Sodium dialysate and ultrafiltration were constant. Protocol II: sodium was linear and ultrafiltration was constant. Protocol III: Sodium and ultrafiltration were linear. Protocol IV (routine): Sodium and ultrafiltration were constant. Using Chi-Square and relative risk, the data was analyzed (P<0.05).
Results: The incidence of hypotension, at the end of the fourth hour of dialysis in Protocol 3, was significantly decreased compared to that of routine method (P<0.05), while at the end of first, second and third hour, this difference was not significant. Muscle cramp in the routine method was more than other protocols (P=0.034). The relative risk of muscle cramp in the routine method was 2.08 times of protocol I, 1.09 of Protocol II, 2.08 of protocol III.
Conclusion: Sodium and ultrafiltration profile is simple and cost-free, and it reduces the incidence of hypotension and muscle cramp during dialysis. Thus, we recommend using sodium and UF profile instead of routine protocol to reduce these effects.

Abstract

Background and Objective: ‌This study aimed to assess nurses' attitude toward the effect of nursing electronic reports on patient care. Material and Methods: This descriptive-analytical study was conducted on 316 nurses in the hospitals affiliated to Semnan University of Medical Sciences and social security organization, 2012.‌ The instrument was a researcher-made questionnaire,‌ valid and reliable. The data was analyzed by Chi Square, Fisher and Phi test.
Results: Forty-seven five point percent of the nurses completely disagreed with the impact of electronic reporting on patient assessment‌ (1.8‌± 1.06) and 54.4% completely disagreed with its impact on the selection of clinical intervention (1.8 ± 1.06). On the other hand , 50% of nurses agreed with the effect of the computer program on patient care (2.2±0.99), 42.4% were satisfied with its effect on patient's diet planning (2.1±1.5) and 40.7% with the computer program on nursing diagnosis (2.01±1.09).There was significant relationship between some of the nurses’ demographic characteristics and their attitude (P‌<0.05).
Conclusion: Based on the results, nurses’ attitudes can be attributed to some defects in the computer programs and to the nurses’ compliance with information technology. To improve the effectiveness of electronic reporting, first, the quality of computer programs of clinical settings should be enhanced, and then changes should be occurred in organizational policies and in electronic documentation. Further, we should consider the impact of Levine's theory of change, indicating the staff’s resistance to technology adoption in workplace.

Abstract

Background and Objective: Breast cancer is the most common cancer among women worldwide. Thus, we aimed to determine the effect of stress inoculation group training on the hope and quality of life in women with breast cancer. 
Material and Methods: This experimental study was conducted on 60 randomly selected women in chemotherapy ward of Razi hospital in Rasht, 2011. The instruments were Schneider hope and quality of life (SF-36) questionnaires. Of 60, 30 eligible patients were randomly divided into control group with no training and experimental group with stress inoculation training. 
Results: Before intervention, the two groups were matched with demographic variables, hope and quality of life score. The results showed that the stress inoculation group training is effective both on hope and quality of life. Furthermore, the effect of this approach on quality of life was more than hope score (P=0.001). 
Conclusion: Stress inoculation group training is highly effective in improving the hope and quality of life in women with breast cancer. To be sure about its permanent effect, further research with follow-up period is needed.

Abstract

Background and Objective: Fatigue is one of the most common side effect in hemodialysis patients. This study aimed to assess the factors associated with fatigue in hemodialysis patients, such as demographic variables, anxiety, depression and quality of sleep. 
Material and Methods: This descriptive-analytic study was conducted on 58 eligible, randomly selected patients of 165 hemodialysis patients referred to Panje Azar Hospital in Gorgan. The instruments were a demographic checklist, Fatigue Severity Scale, Beck Anxiety Inventory, and the Beck Depression Inventory. The data was analyzed by SPSS 17 software using Mann-Whitney, Kruskal-Wallis, Spearman's correlation coefficient and linear regression. 
Results: The mean average of fatigue in all patients was 1.66±4.76. Twenty-six of the participants (44.8%) expressed that they suffer from moderate anxiety, 17 (29.3%) from severe depression and 43 (74.1%) from poor quality of sleep. The relationship between fatigue and anxiety (P = 0.006, r = 0.353) and depression (P≤.001, r=0.525) was directly significant whereas the relationship is not significant for sleep quality. 
Conclusion: Given the high prevalence of fatigue in hemodialysis patients and the impact of multiple factors, we strongly recommend that care providers should consider these factors to improve patients’ quality of life.

Abstract

Background and Objective: It is imperative that the application of appropriate educational methods is of paramount importance to nutrition knowledge and health of pregnant women. Hence, this study aimed at investigating the effect of lecture and group discussion on improving pregnant women's awareness. 
Material and Methods: This peri-experimental study was conducted on 110 pregnant women selected via cluster random sampling in eight villages of Gorgan, Iran, in 2009. They were allocated to two groups of lecture (n=52) and group discussion (n=36). The instrument was researcher made awareness questionnaire, consisting of 22 questions, whose reliability was confirmed by Alpha Cronbach (0.647) for data analysis, we used Pearson correlation, pared t test and t-dependent. 
Results: The mean age of lecture group was 24.65± 5.13 and that of group discussion was 23.56± 14.48. the educational level in 86.5% of lecture group and 94.4% of group discussion was at the level of middle school. There was no significant relationship between lecture group and group discussion. Furthermore , both groups had not any significant correlation with variables such as gestational age, pregnancy history and education level.
Conclusion: Given that the current training programs in health system are adequate, it is a necessity to motivate and promote healthy behavior in pregnant women.

Abstract

Background and Objective: The ideal value of clinical training in personal and professional development, as well as the clinical nursing skills is undeniable. Hence, we aimed at investigating the obstacles of clinical education and strategies for improving the quality of education in Yasuj University of Medical Sciences. 
Material and Methods: This cross-sectional study was conducted on 108 nursing students who had attended at least one term of practical work in Hospital, 2010. The instrument was a questionnaire, including questions related to demographic characteristics and barriers of clinical skills, whose validity and reliability were approved (R = 0.81). The data was analyzed by SPSS 17. 
Results: The main obstacles were absence of learning objectives (85.9%), students' failure to maintain discipline (77.8%), lack of adequate training period (66.6%), lack of appropriate communication between students and personnel (62.1%). There was significant difference between male and female students about the importance of training records of instructors. (P <.001). But students' view was not significantly correlated with their demographic characteristics (P >0.05). 
Conclusion: There are major obstacles in four areas of clinical education that can be effective in students' practical skills. Thus, it is important for education officials to perform some appropriate interventions to provide a suitable clinical setting having education facilities, clinical environments and modified evaluation instruments.

Abstract

Background and Objective: One of the environmental factors bringing about chronic stress is something that is related to occupation. Because mental health improvement of a society particularly in a productive group is a necessity for dynamics and progression of a community, we aimed to examine the relationship between occupational stress and mental health in medical emergency's Male staff of Golestan province, 2012-2013. 
Material and Methods: This correlational study was conducted, via census method, 206 pre-hospital personnel working in 44 emergency stations. The instruments were demographic checklist, Occupational stress questionnaire for nurses and Goldberg general health questionnaire. The data was analyzed by descriptive statistics, Pearson correlation coefficient, ANOVA, T-test, and Tukey Test, using SPSS 16 software. 
Results: One hundred and seventeen (75.5%) of emergency staff had a medium to high level of occupational stress and 145 of them (75.1%) a medium to low level of mental health. A significant inverse relationship was seen between the overall level of occupational stress and mental health. Occupational stress was significantly associated with the variables such as age and type of employment, and mental health with type of employment. Conclusion: Occupational stress can adversely affect clinical practice and mental health in pre-hospital personnel. Hence, the use of some strategies to reduce stress can improve the mental health in that it leads to increase the quality of nursing care.

Abstract

Background and Objective: Cool temperature dialysate has been recommended to improve hypotension during dialysis. Thus, we aimed to evaluate the effect of lowering the dialysate temperature on intradialytic hypotension (IDH) and some interventions in patients undergoing hemodialysis (HD). Material and Methods: This randomized controlled trial with a cross over design was conducted on 50 patients divided into two equal groups. Each group was dialyzed for two periods of 12 HD sessions, once with standard temperature (37°C) and in the next period with cool dialysate (35.5°C). Blood pressure and the related interventions were noted during each dialysis. The Data was analyzed using Chi-square, McNemar, Independent and Paired T Tests. 
Results: Overall, the frequency of hypotension in routine method was 18% and in cool dialysis was 5%. It means that hypotention in cool dialysis is significantly less than that of routin method (P<0.004). Furthermore, the mean of hypotension related interventions in both groups was also significant (P<0.001). 
Conclusion: Owing to the effect of cool dialysate temperature on stabilizing hemodynamic parameters, we recommend using this method during hemodialysis in hypotensive patients.

Abstract

Background and Objective: Role Strain refers to the felt difficulty in fulfilling expectations, complying with the role obligations and balancing the various dimensions of roles. Role strain is a subjective state of distress experienced. The aim of this study was to explain nursing teachers’ perception of role strain. 
Material and Methods: This qualitative study was conducted in 2013, based on conventional content analysis. Using purposeful sampling, 21 nursing teachers were selected from schools of Nursing and Midwifery of the country. The data was collected through Semi-structured, in-depth interviews, and analyzed via content analysis using Granhiem and Landman (2004). The rigor and confirmability of the data were used by Guba and Lincoln criteria. Results: The data was summarized in sixteen subcategories and five main categories. Nursing teachers’ perception about role strain was placed in following categories: Duty interference, Work Overload, Clinical teaching problems, Educational system problems and Students’ problems. 
Conclusion: Given the work-related requirements and role obligations, we recommend that managers and health policy makers be careful about some protective strategies to reduce the role strain.

Abstract

Background and Objective: Subjective well-being, a field of positive psychology, attempts to assess the cognitive and emotional lives. Given the fact that nursing and midwifery students are exposed to stress in clinical settings, we aimed to assess subjective well-being to promote educational programs. 
Material and Methods: This across-sectional study was conducted on 104 nursing and Midwifery students selected via simple random sampling, 2013. The instruments, filled out by students, were a demographic checklist and Kyeez subjective well-being questionnaire consisting of three domains of emotional, social and psychological. We used mean and standard deviation for data description and T-test and ANOVA for analysis. 
Results: The mean score of subjective well-being was 203.20 in nursing and 210.21 in midwifery students. In nursing students, the Mean score in emotional, psychological and social domain was 44.05, 93.40 and 65.74, respectively, and it was 45.72, 94.78 and 69.55, respectively in midwifery students. Gender was the only variable associated significantly with subjective well-being of nursing students (P>0.05). 
Conclusion: Given the high score of subjective well-being and that of its domains, we recommend applying some educational programs with the consideration of emotional, psychological and social aspect of students.

Abstract

Background and Objective: Diabetes may lead to several chronic complications but less was studied about anxiety disorders. Thus, we aimed to evaluate the association between anxiety disorders and the quality of life (QOL) in diabetic type two (DMT2) patients. 
Material and Methods: This descriptive- analytic study was conducted on 330 DMT2 patients referring to panje Azar hospital in Gorgan, Iran. The Data was collected via systematic sampling in six months using demographic checklist, Beck Anxiety Inventory & WHO QOL-BREF questionnaire , and analyzed by Chi square test and Pearson correlation coefficient (P ≤0.05). 
Results: The participants were both males (35.45%) and females ( 64.54%) . The overall quality of life was reported unsatisfactory in 37.3%, satisfactory in 55.5% and highly favorable in 5.5 % of the participants. There was an inverse correlation between QOL and anxiety disorders (P ≤0.001), and the only variable having significant correlation with QOL was previous coexisting disorders (P≤0.005) . 
Conclusion: Anxiety disorders in DMT2 patients can have negative effect on quality of life. Hence, early diagnosis, treatment and prevention may result in improving quality of life.
